Four things: clinical history and a full physical examination of the shoulder and neck, review of whatever imaging you bring (and a request for anything missing), an explanation of the diagnosis with the images in front of you, and a written treatment plan with the real options, including not operating.

Whatever you already have: X-rays, ultrasound scans or MRIs, on CD or on your phone. If something is missing, the appropriate scan is ordered in the consultation itself depending on what is suspected. It also helps to bring previous shoulder reports and a list of the medication you take regularly.
